The city Department of Health is surveying adolescents so it can better understand the effect the September 11th terror attacks has had on their health.

The World Trade Center Health Registry survey will focus on both late emerging and long-term health problems.

The survey, which includes questions on physical and mental health, has two parts, one for the parent and one for the child.

It can be submitted by mail or online with a parent's consent.

This is the third pediatric survey sent out this year, and will target more than 1,300 children under age 18.

Data will be collected through March 2012. To find out more information about the health survey, call the registry at 1-866-692-9827.
